HOW IT WORKS

Participant Referral Process

We understand that navigating the NDIS can feel complex, so we aim to keep the referral process simple and straightforward.

You can begin by completing the referral form. If you need assistance, you can contact us directly via email and our team will guide you through the process.

Working With Us

Where Care Feels Personal, Support Feels Right

Once your referral has been received, we will contact you to arrange a time to meet and discuss your support needs.

If you choose Home Nursing Solutions as your provider, we will prepare a service agreement outlining the supports, services, and care we will deliver.

This agreement ensures clarity, transparency, and alignment with your goals.
